The Case Specialist is responsible for the case management of Addventures members. The Case Specialist will ensure Addventures staff are implementing member ISPs as written and that all individuals are achieving their individualized goals. The Case Specialist is responsible for achieving departmental goals and ensuring programming is operating in an efficient and effective manner.


Essential Duties and Responsibilities

  • Acts as Addventures' primary point of contact for external team members
  • Attends Individual Specific, Behavior Support and all other required team meetings on the behalf of Addventures
  • Authors Addventures Profiles, Skills Developments, and Adult Day Programming Documentation
  • Trains staff on Individual Specific Plans, Behavior Support Plans, Profiles, and documentation information
  • Monitors documentation to ensure members are meeting individualized goals
  • Assists with intake process
  • Assists with Incident Reporting and Major Unusual Incident Reporting protocols
  • Ensures all consents are completed and updated annually for all members
  • Creates and maintains members files (electronic and hard copy) to ensure they meet Licensure requirements
  • Participates in creation, distribution, and collection of quarterly programming calendars
  • Maintains Activity Fund Ledgers for members
  • Collects and documents activity account receipts 
  • Updates leadership on member status changes
  • Maintains master attendance documentation
  • Serves as a backup direct care staff to lead classes, provide supervision, and personal care on an as needed basis
  • Serves as a backup van driver to transport members on an as needed basis
  • Other duties as assigned

  • Associate's degree or equivalent experience required; bachelor's degree preferred
  • Minimum 3 years' experience working with people with developmental disabilities preferred
  • Valid Ohio Driver's license with 5 points or less and proof of insurance required
  • Current CPR & First Aid certificates or the ability to obtain certifications
  • Current Medication Administration Certification or the ability to obtain certification
  • Ability to pass drug test and background checks
  • Able to lift/lower 50 to 100 lbs. in order to assist members with transfers, etc., as needed
