Summary
Activities Assistant will provides day programming to people with disabilities with a focus on working with animals. Will engage persons served in a variety of animal related outings and activities. Services may include assisting persons served with personal care, transportation, interpersonal relationships, community experiences, and other services outlined in the persons served individual service plan. Services for animals may include feeding, cage cleaning, grooming, and arranging vet appointments.


Essential Duties and Responsibilities
• Supervises and assists persons served with interacting and developing relationships with animals.
• Implements, as written, each persons served service plan and behavior support plan.
• Administers medications to individuals as prescribed by their physician, following the Medication Administration Procedure and recording on the appropriate documentation forms.
• Assists the persons served with personal care as specified in the individual service plan.
• Responsible for completing daily documentation forms accurately.
• Responsible for the health and safety of the animals at Constant Companions. Duties include feeding, watering, and cage cleaning as well as supervising the interaction between the persons served and the animals.
• Reports persons served concerns to the Constant Companions Manager.
• Responsible for arranging transportation for the persons served which may include transporting individuals on the Constant Companions van, following all safety guidelines for the vehicle and the lift, and keeping transportation logs up to date.
• Responsible for reporting all Major Unusual Incidents or Unusual Incidents to the Constant Companions Manager and completing incident reports within the appropriate timeframe.
• In an emergency, taking the necessary actions to ensure the person served, staff, and animal's safety by contacting the appropriate emergency agencies (ie."911") and the Constant Companions Manager.
• Supervises and assists persons served to maintain health and safety.
• Notifies the Constant Companions Manager of needed repairs of any equipment used by and/or for the persons served.
• Responsible for maintaining a safe and clean environment for the persons served and animals.
• Assists persons served with simple financial transactions while on outings and provides receipts for such.
• Responsible for obtaining receipts for any purchases made using the petty cash account.
• Other duties as assigned.

Essential Skills
• Must possess skills in the areas of interpersonal relationships, flexibility, problem solving and demonstration of good common sense.
• Is responsible for routine operations and decisions which are within position description, but occasionally requires innovation beyond scope of existing practice.
• Must represent the Constant Companions Program in a professional manner at all times.
• Ability to work alongside and communicate with representatives from the County Boards of DD, residential providers, and the participant's families and/or advocates for the benefit of the persons served.
• Must be able to work and communicate with all levels of staff.
• Displays commitment to the missions, values and vision of Constant Companions.
• Communicates and collaborates clearly and effectively.
• Initiates and maintains positive relations with co-workers, persons served and community.
• Takes responsibility for self-development, professional growth and supports a learning environment.
• Understands and implements the processes pertaining to the specific job.
• Consistently demonstrates best practices while performing the specific job duties.
• Consistently honors all commitments and holds self and others responsible for completion and follow-through on duties.
• Understanding of the Rights of Persons with Disabilities.

Qualifications
• High School diploma or equivalent.
• Previous work experience preferred.
• Basic pet care experience.
• Must maintain current CPR, First Aid, and Medication Administration Certification.
• Must have a valid Ohio driver's license.
• Complete Defensive Driving Certification Course.
• Knowledge of regulatory standards pertinent to the organization.
• Must be able to operate a computer and the willingness to learn to use the agency's software.

Working Environment
Normal working hours: Monday-Friday, 10:00 a.m. - 3:00 p.m. Hours may vary based on program need.

May be involved with public presentation of agency program. May be required to transport persons served in an agency van.
